INDIVIDUAL RECOGNITION


EMPLOYEE OF THE YEAR -

Julie Shaw was selected as the 2016 Haxtun Hospital District Employee of the Year by her fellow employees. Julie accepted the award at the annual Christmas Party Dec. 10th with great humility and grace, just some of the outstanding characteristics that she displays in everything she does. Those in attendance honored her with a standing ovation which she unquestionably deserved. Julie is the Activities Director for the HHD Long Term Care Residents with 9 years' experience in this position. She is very devoted to meeting the specific, individual needs of each one and treats them like family. She purposely focuses on keeping the residents an active part of the community utilizing weekly outings to shop or have coffee downtown, tour the town to reminisce days gone by or invite community groups in for special activities. Making the most of each residents day is a special calling in her life and she loves being the hands and feet of Jesus, sharing his joy with everyone she meets.

EMPLOYEE OF THE QUARTER -

The Employee of the Quarter for the first quarter 2017 is Melinda Stocker Rovetti. Melinda is a RN who works the night shift in the Acute Care Unit at Haxtun Hospital. She was nominated for this award for her willingness to accept additional responsibility and ownership to help ensure training for clinical staff in multiple new project areas. She volunteered for this heavier workload and contributed extra effort to coordinate hands on computer based staff training for the Nihon Koden and Athena projects. In addition to staff training she assisted with identifying work flow, creating templates, the initial upload and conversion, following all the way through to the go live process. Working through these very daunting tasks she has exemplified qualities that strongly support the success of Haxtun Hospital. We are very grateful for her assistance.

EMPLOYEE OF THE QUARTER -

The employee of the quarter for the fourth quarter 2016 is Linda Oosthuysen, MA. She was nominated for this award by two fellow employees. "Linda is overdue in being nominated for this award. She is very conscientious of her co-workers and her patients. She always has a smile and kind word. She recently took it upon herself to give extra personal TLC to a patient in the hospital. She is genuine and caring and is most deserving of this award."
The other nomination states, "Linda is so caring and nurturing with patients and staff. She treats people with the up-most respect. She always shows a positive attitude, even during times of great adversity and conflict. She never speaks about "office politics" and never judges anyone. Linda loves people. She took great care in bathing a challenging patient-giving this patient the kindness and respect the patient deserved. Linda is always trying to learn more to be the best she can be. We are fortunate to have Linda on our team."


EMPLOYEE OF THE QUARTER -

The employee of the quarter for the third quarter 2016 is Angela Wernsman. Angela independently handles the laundry for all departments of the Hospital, a very, very big job. Her nomination states "Angela works swiftly and efficiently and is often seen carrying large bagfuls of laundry. She is mindful of other staff or passers by, always stepping aside to avoid collision. Clean laundry is delivered in a timely manner, spotless and always unwrinkled and folded beautifully. Angela is an amazing woman. Her communication of specific movements with her hands, good eye contact & focus on the moment helps with good understanding and team work. I have also observed what an amazing mother she is to her children." Angela is very passionate about her work and Haxtun Hospital is very fortunate to have her as part of our team.



EMPLOYEE OF THE QUARTER -

The employee of the quarter for the second quarter 2016 is Julie Shaw. Julie is the Activities Director for the Extended Care Unit of Haxtun Hospital but as it states in her nomination, she is so much more than that. "Julie goes out of her way on a daily basis to make everything perfect for the resident's activities, taking extra time to visit with them on a one to one basis. In addition, she always displays a positive attitude and shares a smile with everyone. To emphasize her kindness and caring, she recently took the time from her busy schedule to care for and entertain two very small children in the lobby while their mother completed a medical test that she needed. This is our Julie, sharing her love and kindness with our patients and residents. Haxtun Hospital is very fortunate to have her as an employee and she is very deserving of this award."



EMPLOYEE OF THE QUARTER -

The Employee of the Quarter for the first quarter in 2016 is Cindy Clark, a member of the housekeeping staff at HHD. She received two nominations for this award which together listed 24 very positive characteristics/qualities that Cindy demonstrates in the work place daily. Some of them that speak very clearly about the kind of person she is follow. "Not to good to do anything, always goes above and beyond with whatever she does/no short cuts, always has a smile and a kind word for everyone, always volunteering and going the extra mile to finish a project, always willing to fill for someone in a pinch, will drop everything to help others, passionate, caring, such a wonderful, funny, hardworking person."
HHD is fortunate to have her here, demonstrating these wonderful qualities to patients and staff.


EMPLOYEE OF THE QUARTER -

The Employee of the Quarter for the last quarter of 2015 is Lea White, a Registered Nurse at HHD. She received two nominations for this award. "Lea has been a home grown dedicated nurse for HHD for a long time. She has worked in each position from CNA to LPN to RN and brings with her a wide knowledge base that she is happy to share with anyone. She always has a smiling face, a listening ear, and a soft shoulder for patients, residents, visitors and staff. Lea exemplifies nursing and professionalism in so many ways. She volunteers to pick up extra shifts more than any other nursing staff employee. She does this to provide a sense of stability, routine and normalcy which is especially crucial for the ECU residents. She deserves this award 1,000 times over for her continued compassion, caring attitude and love for her patients, residents, and her chosen profession."


EMPLOYEE OF THE QUARTER -
The second HHD Employee of the Quarter award was presented on Oct. 26, 2015 to Diane Ulmer, a Licensed Practical Nurse in the Extended Care Unit.

"Diane has the level of care that every facility needs. She is an advocate for the residents of the Extended Care Unit and for the facility regardless of the overwhelming paperwork requirements and staffing challenges she has to deal with.
She:
• pays close attention and sees resident's needs that are often otherwise overlooked
• visits personally with residents when they are feeling down
• takes time to make popcorn and deliver it to residents with a smile
• spends extra time with the residents and never makes them feel rushed
• responds to individual interests of the residents
• has the respect of the other staff which builds positive team work and enhances resident care
• has a cheery attitude that radiates to the residents and other staff
• is open to new ideas and doesn't complain when they may require extra time

Diane's compassionate personality and exceptional nursing skills make her an excellent choice for Employee of the Quarter."

EMPLOYEE OF THE QUARTER

The very first HHD Employee of the Quarter award was presented to Steven Smith from the Information Technology Department on Aug. 24th, 2015.

This extremely deserving young man was nominated by a fellow employee for exceptional performance in numerous areas. Some of those include producing high quantity, high quality work throughout his employment at HHD, giving extra effort to complete a project and handle a heavier work load, volunteering for and working on special projects, creative problem solving resulting in substantial cost and time savings to name a few.

Steven is very respectful of others and always willing to help. He is tremendously valued and appreciated by HHD staff. Congratulations Steven and thanks for all you do to make each day brighter for others.


INDIVIDUAL RECOGNITION


GOLD STAR AWARD -

The Gold Star Award for the first quarter 2017, was presented to Mark Evers, Director of Facility Maintenance. He was nominated for the award by HHD employees, a unique and notable nomination. His nomination states, "During and after the recent area wide fire emergency, Mark demonstrated commendable communication, coordination, and leadership while serving as the HHD Hospital Incident Commander. He coordinated with community resources toward the joint effort of local and regional responders. After the danger was past, Mark joined with Command Center resources at the Community Center to assist in ensuring the many aspects of safe closure following the disaster. Mark's participation and knowledge in all areas of emergency preparedness programs served the Hospital most effectively. Furthermore, his knowledge and direction of hospital regulations and oversight as it effects HHD and the community as well as our onsite safety and security standards is quite apparent and appreciated."

GOLD STAR AWARD -

The Gold Star Award for the fourth quarter 2016, was presented to Deb MacCrone, Business Office Billing Specialist. She was nominated for the award by two fellow employees, both focusing on the love and compassion she showed to the Residents of the Long Term Care Unit and staff during the holidays. "The last couple months, I have watched an overwhelming, soft hearted experience from Deb. She has taken it upon herself to bring more sunshine and smiles to our residents for the holidays, which can be a difficult time for them. Thank you Deb for making a heart feel warm, create a sparkle in an eye and a smile of hope with your love." The other nomination states, "Deb has demonstrated excellent care and compassion for our residents. She strives for smiles and happy holidays for everyone. She has thought of each and every resident by sharing many thoughtful gifts and taking time out of her own day to make them all feel loved. Deb has a warm heart for everyone and goes above and beyond to show that. She is a true asset to our work family!"

GOLD STAR AWARD -

The Gold Star Award for the third quarter, 2016 was presented to a Haxtun Hospital RN, Jessica Bullard. She was nominated for this award by one of the physicians at Haxtun Hospital. "Jessica's excellent nursing and communication skills led to saving a man's life today. Jessica was the RN on an ambulance transport of a cardiac patient en route to Greeley when she became aware of evolving ECG changes on the monitor. In collaboration with HHD and EMS staff, she requested a divert to the Fort Morgan Hospital. There, the patient was found to be having an acute MI and the ER staff was able to give clot-busting drugs almost immediately saving the man's life. He was then flown to Greeley where things were touch and go, but as of now, he is doing okay. Good job team and Jessica was the STAR!"

GOLD STAR AWARD -

The Gold Star award for the second quarter 2016 is Coleen
Smith. Colleen is a cook in the dietary department at Haxtun Hospital. Her nomination states, "Colleen pleases people with a gladness in her heart. No matter how busy she is, she always willingly serves a juice for the physicians. If I ask for a little fresh milk for coffee, she graciously gives permission. I am also so thankful for the delicious meal she put together in haste for a clinic outpatient who needed some TLC while waiting in a hospital room over the lunch hour.

GOLD STAR AWARD -

The Gold Star Award for March was awarded to Laura Beard.
Laura is a member of the housekeeping staff at HHD.
Laura was nominated for this award for going above and beyond to accommodate moving multiple patients to facilitate a patient transfer. This complicated process occurred near the end of her shift but she gladly agreed with a smile and cheerfulness to stay and help. She stayed and worked until she was sure that each patient had a clean room and a clean bed plus making a room ready for an additional admission, which actually occurred later that night.

GOLD STAR AWARD -

The Gold Star Award for Feb. was awarded to Jesse Cochran. Jesse is an LPN in the Extended Care Unit at Haxtun Hospital.
Jesse received this nomination for graciously accepting the role of making sure that when faced with end of life, our residents and/or family members receive the comforting gift of a prayer shawl. Her compassion and attention to the individual needs of the residents in her care were only two of the reasons she was chosen by her co-workers to personally deliver this gift from HHD staff.

GOLD STAR AWARD -

The Gold Star Award for Jan. 2016 was awarded to Breanna White-Nies. Breanna is the Pharmacy Tech at the HHD Pharmacy. Breanna received two nominations for this award. Some of the qualities that were listed to describe her are caring, generous, kind, friendly, and helpful. "She is dedicated to taking the time to listen to customers to make sure they understand the specifics of their medications. Breanna also has a passion for and a commitment to the community." Two specific instances were mentioned in her nominations that demonstrate this commitment. She baked and delivered 4 dozen cookies to an elderly, disabled man because he wanted to take them to his church for Christmas but couldn't do it on this own. She has also connected with and redirected a wandering ECU patient multiple times, doing this by having a conversation with him and never making it seem like a chore.

GOLD STAR AWARD -

The Gold Star Award for December was awarded to Rhonda Menard. Rhonda is a Billing Specialist at Haxtun Hospital. The Extended Care Unit at HHD is home to approximately 20 residents and one of the special enhancements in their living area is an aviary. "In the absence of the aviary being cleaned regularly, Rhonda took this responsibility on herself as well as making sure the birds were fed and that the residents continued to have this pleasant diversion. For Rhonda, just seeing the smiles on the faces of the residents is a reward. This is just one example of how Rhonda provides compassion and caring for our residents. She is always willing to go the extra mile to make their day more pleasant."

GOLD STAR AWARD -

The HHD Gold Star Award for November was awarded to Ira Kinchen. Ira is a Lab Technician at Haxtun Hospital. "Ira consistently is more than willing to help when another department is in need. He often assists the residents of the Extended Care Unit when the lab has down time. He has run activities for the residents when the Activities Director is unavailable, participated with activities, and assists residents to the dining room. I have repeatedly seen Ira's willingness to help nursing in ER situations. I believe Ira's interaction with the residents and staff makes a difference. He never has a that's not my job mentality and always has a positive attitude."

GOLD STAR AWARD -

The HHD Gold Star Award for October was awarded to Terry Hovel. Terry is a floor technician at Haxtun Hospital. "Terry works very well with others in all of the floor projects and he is always friendly. He did an awesome job on the Dining Room floor in the Extended Care Unit, it looks so much nicer than before."

GOLD STAR AWARD -

The Gold Star Award for September was awarded to Sheri West. "Sheri always goes above and beyond for our department by always making each patient feel important and like they are the only one that day. Most recently, Sheri made a literal difference in a patient's life by answering the phone, recognizing the complication, staying calm and getting appropriate help in the correct time frame. Not only did I recognize this from the outside, the patient identified Sheri as a lifesaver as well. This is so important to note because so many people that are not doctors, nurses or Medical Assistants feel like they don't "help" that much. YOU DO! You change lives just with a calm voice, a welcoming smile and assisting any way you can. Thank you for being such as asset to our team! Very much appreciated!

GOLD STAR AWARD -

HHD made the first Gold Star Award presentation in Aug. 2015.
The first Gold Star Award for HHD was actually shared by two employees nominated for their contributions to the remodel of the PT Clinic. Bryan Klassen and Patrick Dick were nominated by a fellow employee for exceptional performance in many areas. Some of those include "creativity and willingness to work outside the box, sensitivity to the need of patients and maintaining the continued operation of the PT clinic during construction. They balanced the details of this project integrating a great sense of humor and simultaneously answering calls from other departments for their services. RECOGNITION and CELEBRATION OF ALL STAFF

STAFF BARBEQUES - A minimum of 3 all staff BBQ's are held in the Hospital courtyard each year. These are typically scheduled to coincide with Hospital Week/Memorial Day, Independence Day, and Labor Day.

BIRTHDAYS - Monthly recognition of staff birthdays with birthday cake served in dining room on the third Thursday of each month.

MISC. - Spontaneous "events" are held throughout the year and may include serving Ice Cream, Popcorn, etc. to all staff.
