NURSE NAVIGATOR

Under the supervision of the Specialty Clinic Manager this position provides direct patient care and comprehensive coordination of care for patients and their families/caregivers. The Nurse Navigator acts as an advocate to assist the high-risk patient population in navigating throughout internal and external systems in order to facilitate the diagnosis and treatment process. Applies the nursing process through assessment, planning, implementation and evaluation of the patient in order to provide the optimal level of patient care.

Qualifications

  • Current Colorado RN License required
  • Basic Life Support (BLS) required
  • Advanced Cardiac Life Support (ACLS) required
  • Pediatric Advanced Life Support (PALS) required
  • Minimum of three (3) years of clinic nursing experience related to specialty area required
  • Previous experience with emphasis on disease management and patient education required
  • Experience in case management, nurse navigation, and/or care coordination preferred

Essential Job Duties & Responsibilities

  1. Performs outpatient counseling & management.
  • Functions as a clinician in a consultative and supportive role to the providers in managing patients with chronic conditions including but not limited to: Diabetes, Pulmonary Disease, Cardiac Disease, and Renal Disease.
  • Provides the follow-through and appropriate support to assist patients with managing their conditions.

 

  1. Provides assessment, disease management, education, training, and other clinically based activities to coordinate care among providers, clients, and the community.
  • Provides direct patient nursing care for specified patients as prescribed.
  • Performs nursing assessments and triage of patient care needs for new and ongoing patients.
  • Identify members for specific case management and/or disease management activities or interventions.
  • Assists patients in understanding diagnosis, treatment options and resources available internal and external to the organization.
  • Serves as liaison in planning care between patients, caregivers and multi-disciplinary health care providers.
  • Provides education and informational resources to patient to optimize continuous and comprehensive care.

 

  1. Collaborate with healthcare providers and community partners to develop an evidence-based, comprehensive individualized plan of care.
  • Assists to oversee the planning, scheduling, and implementation of day-to-day clinical activities and procedures for patient/family.
  • Develop, document, and monitor patient/family response to plan of care, and revises the plan as indicated.
  • Provides patient self-management support and tools with a focus on empowering the patient/family to build capacity for self-care.
  • Coordinates care services with other healthcare providers to remove barriers to achieving goals, including but not limited to making physician appointments, assuring prescriptions are filled.
  • Completes timely follow-up including PCP or specialist follow-up appointments, symptom assessment, providing education utilizing teach back methodology, coordination of care, and problem-solving potential and/or actual barriers.
  1. Promotes and ensures patient safety in performance of all responsibilities.
  • Functions as a Registered Nurse as requested in the Outpatient/Specialty Clinic environment.
  • Administers medications, treatments and procedures in a timely and safe manner according to the health care provider orders and nursing policy, including appropriate performance of standard precautions, using two patient identifies, and performing patient risk assessments every shift.
  • Demonstrates proficient technical/clinical skills and operational knowledge of equipment.
  • Utilized proper body mechanics, transfer/lifting techniques to minimize fall risk to patient and injury to self.
  • Implements infectious disease controls as appropriate including isolation procedures, personal protective equipment (PPE), proper use of hand washing and gloves.
  • Maintain a clean and functional environment including cleaning and disinfection of exam room, equipment and storage area. Disinfects equipment and instruments using appropriate solutions following sterilization procedures.
  • Follows proper procedures for wasting narcotics and disposal of sharps and hazardous materials.

 

  1. Assists in the examination of clinical program information.
  • Collects data and prepares various reports pertinent to department.
  • Participates in quality improvement initiatives as well as outside committees.
  • Participate in multidisciplinary team conferences, and participates in overall program evaluation of processes, identifies problems, and proposes/develops process improvement strategies to enhance care management services.
