Each and every one of our residents has a unique health history, and we recognize that care plans need to be individualized for each resident. Our Haxtun Health Extended Care team is made up of physicians, registered nurses, physical and occupational therapists, a registered dietician, a social services coordinator, a restorative aide, certified nurse aids and a pharmacist. The team builds a care plan for each resident based on individual needs for diet, exercise, medication management and overall physical and mental health.

Our restorative aide is here five days a week to help maintain patient range of motion, assist with exercising and encouraging/assisting with maintaining independence. All of our residents are seen by a provider at least once a month for health evaluations and their care plan is discussed weekly with the entire care team.
