Haxtun Health urges community members to participate in its Community Health Needs Assessment (CHNA) survey, a critical tool for understanding and addressing local health priorities. The survey aims to identify key health challenges, barriers to healthcare access, and inform the development of targeted programs and services.

“Your feedback is crucial in helping us understand and address the unique health needs of our community,” stated Haxtun Health representative, Jennifer Wernsman. “We want to know, ‘What does our community need to maintain optimal wellness or how can we help them get back to the healthy lifestyle they once had?'”

The CHNA focuses on identifying the most pressing health issues within the community. It also aims to uncover obstacles like transportation issues, lack of insurance, or cultural barriers that prevent access to healthcare. Furthermore, the data collected will be used to design and implement programs that directly address identified needs.

Participation is vital as it allows Haxtun Health to create programs and services tailored to the community’s specific needs. All responses are confidential, and all questions are optional. However, providing more information helps Haxtun Health gain a more complete understanding of the community’s health needs.
